2024/25 West Middle School Athletics Information Sheet

Final Forms: The BC Athletics department uses a platform called Final Forms to register all athletes for our athletic programs. Parents will need to first create a profile and then register their child under their account through Final Forms. Once registered on Final Forms, athletes can select the sports that they are interested in participating in each year. When a sport is selected, families will begin receiving communication from the head coaches of each sport as the season approaches. If a sport is not selected on an athlete’s account, they will NOT receive any communication from the coach. Please select all sports that you may have interest in, so that you don’t miss out on communications. You can deselect a sport at any time if you decide later not to participate.

Sports Physicals: MHSAA Sports Physical Form
Blank forms are also available in each school's main office.

Please complete the form with all signatures by parent, athlete and licensed physician. Failure to submit a fully completed form will delay eligibility for athletes to begin participation. All physicals must be turned into the main office or uploaded to the athletes Final Forms account before a student is allowed to participate in each sport.

Please note, any physical completed on or before April 15, 2024 will be deemed expired and will NOT be accepted for participation in the 2024/25 school year.

Pay-to-participate: The fee at Byron Center West Middle School is $50 per season with a max payment of $150 per calendar year. If a child receives free/reduced lunch the fee is waived. The district family max is $300 for families that have children playing at both the middle and high school.
